摘要
摘要目的探讨宫腔镜下粘连电切分离术后预防再粘连的方法和疗效.方法选取２０００年１月至２００９年１２月宫腔镜确诊的宫腔中、重度粘连患者２２例为对照组,术后采用传统的宫内放置节育环和常规人工周期治疗;２０１０年１月至２０１４年１２月确诊宫腔中、重度粘连患者患者３８例为实验组,术后采用宫腔内放置水囊导管＋几丁糖凝胶＋戊酸雌二醇人工周期的综合治疗.术后３个月复查宫腔镜,根据结果对症治疗,第三次月经干净后３天宫腔镜检查判断疗效,所有患者随访３－６个月.结果实验组患者治愈率达７１．０５％(２７/３８),而对照组患者治愈率仅２７．２７％(６/２２),两者差异有显著性(P＜０．０５);实验组患者总有效率达９７．３６％,(３７/３８),而对照组患者仅５０％(１１/２２),两者差异有显著性(P＜０．０５).结论综合方法明显减少中重度宫腔粘连术后再粘连的发生,值得临床推广.关键词宫腔粘连球囊尿管几丁糖凝胶abstractobjectivetoevaluatetheadhesionunderhysteroscopyelectricpostoperativeadhesionpreventionandseparationmethodsandcurativeeffect．SelectionmethodsbetweenJanuary２０００andJanuary２００９inFebruaryhysteroscopyinthediagnosisofuterine,２２patientswithsevereadhesionasthecontrolgroup,postoperativeusingconＧventionalintrauterineplacedcoilandconventionalartificialcycletreatment;BetweenJanuary２０１０andDecember２０１４inthediagnosisofuterine,３８patientswithsevereadＧhesionspatientsasexperimentalgroup,postoperativecatheterplacedintrauterinewatersac＋＋chitosangelpentanoicacidestradiolcomprehensivetreatmentofartificialcyＧcle．Postoperative３monthsreviewhysteroscopy,symptomatictreatment,accordingtotheresultsforthethirdtimemenstrualclean３daysafterhysteroscopyexaminationj(udgingcurativeeffect,allpatientswerefollowedupfor３to６months．Resultsthecureratewas７１．０５％(２７/３８)patients,whileonly２７．２７％ofpatientsincontrolgroup２６/２),therewasasignificantdifferenceinsex(P＜０．０５);Totaleffectiveratewas９７．３６％,andtheexperimentalgrouppatients(３７/３８),comparedwithonly５０％incontrolgroup(１１/２２),therewasasignificantdifferenceinsex(P＜０．０５)．ConclusioncomprehensivemethodissignificantlyreducedmoderatelyseverepostoperativeadheＧsionagKaienywcoarudssedbyintrauterineadhesions,worthclinicalpromotion．causedbyintrauterineadhesionsballoonurinarytubechitosangel中图分类号R７１９文献标识码B文章编号１００８－６３１５(２０１５)１２－００７７－０２
